A compact and versatile robotic platform for precision microsurgery

The Challenge

Cochlear implant results depend on threading an electrode array into the cochlea without damaging the delicate structures inside, which is beyond what a surgeon’s hand can control. The resulting trauma destroys the hearing patients still have, so surgery is withheld until hearing is nearly gone. Most eligible patients go unimplanted.

The Opportunity

The Miles Insertion System controls every stage of the insertion mechanically, while real-time impedance sensing reports what is happening inside the cochlea. Trauma falls to non-significant levels, so patients keep the hearing they have and can be implanted years earlier. Compact, fits existing workflow, works with every major commercial electrode array.

The Venture

Business Model

Capital sale or placement of the robot into hospitals and surgical centres, then recurring revenue from a single-use insertion consumable on every case, plus service contracts. We are also exploring co-marketing with implant manufacturers, who benefit directly when hearing preservation improves candidacy and outcomes.

Competition

Existing systems cover only part of the insertion and advance blind at a fixed speed. We control every stage of the insertion mechanically and sense what is happening inside the cochlea in real-time. We also work with every implant.

Traction

TRL 5 with a PCT filed in November 2025. We work with UCLH and UHB, and collaborate with Cochlear Ltd. £1.5m non-dilutive raised, including a £1.1m NIHR i4i PDA award, a Wellcome MedTechONE Translation Fellowship and an RAEng Enterprise Fellowship. Surgeon and buyer interviews complete.
